Enrollment Application

Please submit the form below to apply to have your puppy enrolled in our training school. We cannot guarantee you a spot but this is the place to start and we will contact you if we are able to invite your puppy into our school. All health-related requirements and transportation will be the responsibility of the puppy owner unless special arrangements have been made with Puppy Intelligence. The first week's training cost will be due as a deposit before training can begin. Please contact us for details.
